How do i get a dummy (pacifier) off a 3yr old?

my little brother just turned three and his dummy is giving him a rash around his mouth and we know it really hurts him and we put cream on it but it keeps on coming back bcoz of his dummys... we tell him that there hurting him that his to big for em but he just gets really upset... any easy ways to get it off him please???
